New Home Health Nurse Training: A 10-Part Webinar Series
Working as a nurse in a home health setting involves a number of areas that a nurse experienced in any other setting has likely not been exposed to, such as homebound status, case management, the OASIS document, and safety in treating patients at home. For a home health nurse to be successful, it’s critical that he/she gain knowledge in these fundamental areas. This series is intended to provide an overview in many of these basic topics a nurse new to the home health environment will encounter on a daily basis. Each registration will include the online recording of the session that will be available for unlimited access. If you have missed any of the sessions, you may register for the recording. This is a great educational series to add to your on-boarding library!

The session will include examples of documentation that supports and does not support homebound status as well as explore the impact of driving, trips to day care and other situations on the determination of homebound status.

Objectives:
1. Identify the official Medicare definition of homebound status.
2. Describe real-world examples of documentation that illustrates homebound status
3. Define Special situations and how they impact homebound status including driving, absences from home, patients who attend day care or outpatient services.

Case management is the key to providing cost effective, efficient and quality home health care. The session will also feature some real examples of home health documentation submitted to intermediaries to analyze and determine whether they meet requirements.

Objectives:
1. Identify the 4 key components they must be in every home health note.
2. Analyze sample chart entries to determine if they support home health services.

"Beneficiary Notices"

Part 6: Beneficiary Notices
February 23, 2012 • 12:00pm – 1:30pm PDT
Presented by: Judy Adams, RN, BSN, HCS-D, COS-C

Since 2001, home health agencies and other health care providers have been required to provide official notices to Medicare beneficiaries at the initiation, reduction and termination of Medicare services. Despite the fact, these requirements have been in effect for a decade now, there continues to be confusion about when and which form needs to be provided. Beginning in 2011, the government began to require copies of the appropriate beneficiary notices that must be submitted for review along with other aspects of the medical records in response to Additional Documentation Requests. This session will focus on the requirements and appropriate completion and delivery of the Home Health Advance Beneficiary Notices and Expedited Determination/Notice of Medicare Non-coverage forms.

Objectives:
1. Describe the triggering events for issuing the HHABN.
2. Identify the proper use and way to complete each of the three versions of HHABN forms.
3. Define the use of the two types of Expedited Determination/ Notice of Medicare non-coverage forms.

"Care Coordination: How to Do It and Document It"

Part 9: Care Coordination: How to Do It and Document It
April 5, 2012 • 12:00pm – 1:30pm PDT
Presenter: Rebecca Friedman-Zuber, MA, BSW

Care coordination is both a critical long-standing home health requirement and a hot new buzzword in home care. A lack of care coordination among disciplines is always one of the top 10 deficiencies cited by Medicare surveyors nationwide. The tag numbers associated with care coordination and its documentation—G143 and G144—are among those identified at Level 1(Highest Priority) in the new Medicare survey process. Home health nurses must understand what is required, why it is important, and how to document these activities effectively in order to be successful in the home care industry.

Objectives:
1. Discuss the regulatory and the practical reasons why care coordination is critical in today’s home health agency.
2. Discuss strategies that will help nurses improve the frequency, content and outcomes of their care coordination activities both within the agency and with external entities providing care to their patients.
3. Discuss options for documentation of care coordination activities, including the elements that make approaches more and less successful.
