PROGRAM OVERVIEW
Reducing a home health agency's rehospitalization rate, and improving the star rating is hard! Home Health is seeing more acute patients in the home over the last two years, and in 2023, CMS is changing the hospitalization measure to include observation stays! Sometimes it feels like we can't make an impact on such things-- but home health agencies have- and will continue to make a huge impact on our patient's quality of life- and enabling our patients to recover at home! This webinar breaks down the data, in detail, to determine where our resources are best spent- and then how "going back to basics" can significantly move the needle on rehospitalizations, as well as the five star.

Program Topics:
  • Describe how case management can make an impact on both rehospitalization and five star ratings
  • Define the 2023 updated hospitalization rate (PPH)
  • Identify how the items for 5 star/Hospitalization/HHVBP overlap (and where they are different!)
