On-Demand Discharge Function Score = Outcome Scores and 2026 Reimbursement Webinar
SESSION OVERVIEW
In the proposed rule, the Discharge Function Score (DFS) will be 15% of the OASIS measure weights in 2026, but with the addition of three M questions totaling an additional 7% the overall functional elements of the OASIS increase to 22% of the 40% that OASIS elements affect overall reimbursement. You will learn the overall importance of this and why OASIS scoring needs to be more accurate than ever.
Session Objectives:
- Understand the relationship of the Discharge Function Score to HHVBP and how specific OASIS items impact the outcomes, including the functional scores as well as nursing based scores such as Improvement in Dyspnea and Improvement in Oral Med Management.
- CMS applies risk adjustment to this measure. Learn what this means and how individual and unique patient characteristics may impact performance scores.
- You will gain understanding of how to apply best practices to your scoring, care planning, and documentation to accurately reflect the progress of your patients, emphasizing care coordination among disciplines to further improve accuracy.
