2025 Home Care Awards Call for Nominations

2025 CAHSAH HOME CARE AWARDS 

We invite you to nominate caregivers, leaders, clinicians, or advocates who consistently demonstrate excellence and positively impact the lives of others. 

CAHSAH honors exceptional home care professionals with the CAHSAH Awards Program, recognizing those who provide outstanding service and quality care. These awards highlight individuals who represent the essence of home care, putting others' needs first.   

All awards will be presented at The Joseph H. Hafkenschiel Awards Ceremony Tuesday, May 20th during the 2025 CAHSAH Annual Conference & Expo at The Westin Rancho Mirage Golf Resort & Spa in Rancho Mirage, California, May 19-22, 2025.  

CAHSAH'S AWARDS PROGRAM:

• The Lois C. Lillick Award, honoring an individual who has made outstanding contributions to the continuing development of local, state, and national home care associations.  Download Application.    
• The Physician of the Year Award, recognizing a physician who consistently excels in providing and utilizing home care, and optimizing the role of home care in the health care system.  Download Application. 
• The Lillian H. O’ Brien Home Care Supervisor Award, honoring a first-line home care clinical or office supervisor who has made outstanding contributions in motivating staff to achieve excellence in home care service.  Download Application.
• The Home Care Service Award, honoring an individual directly involved in day-to-day home care, who has consistently provided outstanding service to patients in their homes.  Download Application. 
• CAHSAH Ambassador Award, honoring an individual who has made a special contribution to
  the home care industry.  Download Application.
• Support Service Award, honors a support service individual who demonstrates unique behaviors or practices which support excellence in home care, and a consistent commitment and dedication in promoting home care.  Download Application.
• The Joan Baier-Garland Home Care Nursing Scholarship, provides support to students in good standing in either an AA , BSN or Masters Nursing Program who have demonstrated a commitment to home care and to the community. Scholarship amount: $2,500.  Download Application.
